Lost data connection please help!!

I have an omnia with cf03 and i have had zero problem untill today. Out of no where my data connection stopped working and when it tries to connect a screen pops up asking me for username password and domain. I tried *228 with both options and nothing numerous soft resets aswell. Im trying to fix this any other way then a hard reset please help!!
