This is exactly what mine looks like . Manufacturers date is 11-10-09. I have had my phone in a case logic leather pouch. Never dropped and not a scratch.

You should not need any insurance for something like that. 1 year warranty should cover repair or replacement.

For me it's the Start key that is messed up. It's sunken in. I first noticed it had somehow slid onto the Send key (not sure how), then I popped it back into place, now it's sunken in.

Sorry about the issues I hope a tech can fix it for free.
The TP2's hard buttons have an INCREDIBLY horrible quality.

No, but my buttons do slide around. It's as if there's a sticky substance below them. Its also hard to know when I actually press the button.
