This isn't isolated to custom roms. I am using stock 3..(not the latest) rooted using unrevoked. I have this error all the time when updating apps from the market. The only solution is to unmount then update. It's annoying but the only solution I have found so far.

I had this issue too except I didn't get an error message. The app would download but never install. I tried to unmount which worked, thank you! However, I also noticed that everything on my home screens was duplicated. By this I mean if I deleted a widget, shortcut or whatever, it would still be there. I would have to delete it 2 or 3 times before it was gone. Moving an item would result in 2...the original and the one I moved. My only guess is the dups were created when I restored my back-ups. After I got rid of all the dups and a reboot, my install problem went away.

If you do not want to flash a Rom clean, I would at least go into revocery and wipe everything. Wipe Data, Cache, Davlik VM. Let it all get recreated.

If it were me, I would would install something like Sprint Lover's All-In-One Rom since it is very stable and clean. You have to get to high ground with these issues.

if it happens again just wipe market cache

Having problems updating and installing some apps and get the installation error, unknown -18 reason code too and just wanted to say thanks Geeks for having this information available for me!!!

It worked! The first method worked on my HTC Incredible:
