Quick Answer:
You need to program your phone (*228, option 1)

Detailed Answer:
When you receive a new phone or (sometimes) after hard-resetting your phone you will need to dial:

*228, send, option 1

...to reprogram you phone. This programming is through the Verizon network (ie - you are making a phone call to do this and programming over the air) and should set the username and password for you.

My FULL recommendations if you still have problems:
  1. Backup any data from your phone (pics, files, etc)
  2. Hard-reset your phone. A hard-reset will essentially reload the OS and set your phone to factory default settings (like you just took it out of the box). You will need to check your manual or Google etc. to find out how to do this.
    • Treo: While holding the red End button, press and release the reset button (under the battery cover ...use your stylus), you will then see some red text on the screen asking if you are sure you want to proceed, your call.
    • XV6800: (a little tricky) hold both soft-key buttons (the ones with the white dot) and press and release the reset button on the bottom of the phone (use your stylus).
  3. Reprogram your phone: Dial *228, press Send, choose Option_1 when prompted. You will see a message letting you know if this was successful.
  4. Assuming your account has data enabled, you should now be able to connect. Although you may want to verify the username and password others have mentioned if it still doesn't work.
  5. If it's still not working, you will need to contact Verizon since you probably have a hardware problem (or a software problem that you aren't going to be able to fix). Basically if it's still not working, you are probably just wasting your time now, so call Verizon.
NOTE: I know for a fact that the support techs don't always know what they are doing. Basically their answer is always to replace the phone (probably at your own expense) if they can't figure it out. This works fine if your phone is still under warranty or you have it insured. For those of you who would have to pay for it yourself, I'd check to see if there is a newer version of the software for your phone and get that installed (either on your own or at the local store). I know that the Treos have updates to install that you can do on your own if you are willing.

Like Rob stated earlier I only had this problem when they shut off my internet. Call and double check that they turned it on. As far as verizon goes, just tell them what they want to hear.

If you ever find the stock ROM please email or pm me cause I need it too.

Call in and say your phone has suddenly started having authentication problems. Tech support will reset some items on their end and you should be good to go.

I had this problem also and VZW was completely clueless about fixing it. Just like everything in this thread mentioned, they focused on the username/password entry and then said I had to hard reset the phone to fix it.

I kept getting the error message like about something being erroneously setup or something, followed by username password incorrect. It was the major reason for why I upgraded to 6.1. The upgrade didn't fix it and I was totally upset thinking I had to buy a new phone. I was about to cancel my evdo when i ran into the fix. Like mentioned above you need to reprogram your phone. The easiest way to do it is to dial *22899 and press send on your phone. It will do two things. It will program your phone and update your PRL. After I did that it's worked flawlessly.

Here's the really weird thing that I also got fixed by doing this as a bonus. My MMS started working too. I went through two refurbished phones trying to get that to work and all I needed was to reprogram the phone.

One more thing. I also upgraded my radio firmware to 1.41 as well. My phone had version 1.10 on it. If you need help doing that let me know or search for it on here. It's real easy to do as well.

had the same problem. i deleted the connection settings all together and started over using these settings.

Server name : Verizon Wireless
Dial Number : #777
left the user name and password and domain blank then hit finish
works perfetly now :)

Yes!! my verizon data wasn't working and kept saying "wrong username and password" because I flashed my rom, but all I had to do was go into ##778 and edit with security code 000000, then go menu - security and change the username to
(phone #)@vzw3g.com and password as: vzw, then my data worked perfectly!!
