Don't try the Apache stuff (diff hardware), and the other ROMs are all based on the same things so its not worth trying. Some A2DP headsets don't have an auto-initiate function (the i.tech R35 for example), because most A2DP devices will control the initiation.
My wife's Samsung M510 connects to my i.tech R35 just fine by turning on bluetooth when playing music, but every WM device needs to be manually connected (go to bluetooth settings, find the device and click "set as wireless stereo"). However my Logitech A2DP headphones auto connect just fine to every WM phone just by pressing play on the headset.

Bottom line: Its your car stereo, not your phone. The stereo is probably one of the many that lacks the auto-initiate step. What you can do is write a MortScript to do the manual connection for you and set it to a hardware button. That's what I did for my i.tech. Or, get a Sony stereo!
