I've spoken with numerous VZW techs. Some have tried the "magic form" to request that my MEID be added, others have called the IT dept, etc. All are coming back with the exact same story: VZW never really opened up their network. When I mention the big press releases about their open network that they circulated last July, the techs say "yeah, but that was never approved".
Here's my request to this group: If you find a knowledgable tech who is capable of adding the MEID, would you please get his/her email and PM me?
