Looks like you installed the action screen cab. You should be able to got to settings > system > remove programs to remove.

I like the app!!! It has the quick 9 things I do most of the time - bluetooth most of the time.

EDIT: just saw that you tried that. You could do a hard reset. Or maybe it is time to try your luck with a new ROM...
