I don't know of a "program" that can increase volume. But there are hardware based solutions.

If you want louder speaker volume, the Motorola T505 is the way to go. Not only does it have a VERY LOUD built in speaker, it also can send a signal to your FM radio. So you can do things like play the audio through your car's stereo system. Which gives you the power of your FM stereo's volume controls.

If you want louder headphone volume, the Jabra BT3030 is a good choice. I've found the volume controls on it boost the volume pretty well. Its 3.5mm jack can be used to listen with headphones of your choice if you don't like the ones provided with it. It's been working well with my noise cancellation headphones.
