The application in your picture is LVM Time. You just need to set it up to match the picture.
Go to Settings->Today->Items (tab), select LVM Time and tap Options
Look through the tabs at the bottom to set the items to be displayed and the alignment.
If you want more options try rlCalendar (freeware) They seem to be changing the name to rlToday. rlClock is an older version that just does time and date. rlCalendar/rlToday is completely customizable with clocks, calendars and program launcher. Look in their forums for examples.
