I have seen this with 1.5 and now the 1.6, but I can't nail down when it's happening to me... but it does happen. I thought it was that I had multiple profiles enabled with PA but once I upgraded to 1.6 I have not messed with the profiles (except tweaking some ringtone settings and backlight settings).
What is happening to me is the phone slider pops up and the volume cranks all the way up. It has happened when the phone is sitting on my desk or in my holster.

Yea now its happening when I get a text message too. I'm going to have to try and go without the phone alarm or just go back to 1.4 or something because its annoying when I'm in a meeting and my phone check for emails and I hear this loud ring......
