If all else fails, get the official ROM for your carrier and flash with that:

Disconnect the phone from USB.
Put the phone into bootloader mode. (Hold power + record buttons, while doing a reset.)
Connect the phone to USB.
Run the carrier's ROM installer.

On Vista, get Windows Mobile Device Center 6.1. The one that ships with vista (6.0) is pretty useless.
