I forgot to mention that on the next screen following what I described above that under EAP type: Smart Card or Certificate

The resulting error message is:

Cannot log on to the wireless network. This network requires a personal certificate to positively identify you.

At this time I don't have the certificate installed. However, I did before and it was still asking for a username and password. Again, any and all help is appreciated.
