I don't know if I am supposed to answer bugs in here but here is my experience with one of the adapters.
ROM: Preview 7
Carrier: Verizon
Programs: a lot
I just purchased my adapter from Amazon (bought 2) and have tried S2P, HTC audio Manager, WMP, 2 different headphones (ipod and generic) and both adapters I bought and I don't have any problems with any of the above situations.
I think there may be something wrong with your adapter or settings.
