I had that exact issue. I have a xv6800 and I had flashed it to the 5067 no2chem rom. I had to return the phone so I returned the phone back to the original rom and bootloader. Once that happened I could no longer make data calls. (bad username or password error) I screwed with the phone for a while and ended up with the error code 11. The phone worked fine other than data calls. I called verizon and the guy ended up removing my data plan then recreated it and all was well. Two things to note, the guy said he wasn't supposed to do that (I'm not sure if everyone will have this capability...you might need to go through a couple of techs) and I had to recreate my wireless sync account logon...no big deal though.
