If you're on Sprint (can't speak for other providers), the username/password/domain fields should be blank. Your problem is the dialog rules are dialing #xxx numbers as international calls.

You can either Turn dialing rules off or adjust the international calls rules.

To turn Use Dialing Rules off:
From Settings/Connections select the Connections icon.
Select the Advanced tab.
Push Dialing Rules button.
Uncheck UseDialingRules.

To adjust International Calling rules:
From Settings/Connections select the Connections icon.
Select the Advanced tab.
Push Dialing Rules button.
Either edit an existing rule set or create a new one (For users who don't call or travel internationally, you'll want to edit Mobile)
Change the Area code to your home area code
Push the Dialing Patterns button
Remove the +E from 'For international calls, dial:' rule
(If you don't call or travel internationally, you might consider removing the +E from local calls and long distance calls and then leave Mobile as your active dialing rule set)
