You do not need a stock Sprint ROM first, I ran the unlocker while using Ghetto's 6.1 ROM.

You MUST run the 2.31 unlocker, otherwise you will brick your phone.

I did it with Vista last night and on XP this morning, so I don't think it matters. On Vista, just make sure you have the 6.1 version of the mobile device center.
