Since your doing all the steps right... Gotta be the recovery isn't wiping all the way...
If you just wipe the factory method it doesn't wipe everything unless it was set up that way. Have you tried any other 2.17 builds? Other roms that are built of 2.17 and had the same issue?...
You could try another recovery or try to find a superwipe zip to use in recovery which will wipe what TWRP doesn't....

That was a documented issue with the inicial release of TWRP 2.0
It wouldn't wipe properly.
They even pulled the ones for the og EVO cause it wouldn't even wipe at all from what I've read.
That said, they did release a maintenance "update" of the one for the EVO 3d.
I have had no issues with it.
It is available on their web site.
It's TWRP 2.00.001 or something like that.
If you are using the inicial release, I highly recomend using the updated one.
That may sve your issues.
