To answer your specific question-you dont need to run (restore) the backup unless you want to go back to the ROM you backed up.

Like KIOWA said-its a failsafe/restore point in the event something goes wrong when you flash a custom ROM or a mod.
You can flash a custom ROM and then back THAT up if you want before trying mods or themes on it.
Backup as often as you wish but those backups are large files and will fill your SD card quickly. You really only need to keep one on your card, Custom or Stock, as a safety net.
Nands come in real handy when it comes to themes. You can have your un-themed ROM backed up and then try themes on it. Dont like the theme? Then restore your backup and start over.
