Google voice w/Sprint Integration does this. You can route calls to multiple numbers, even turn off the "main" phone if you'd like.
Without integration, you'd need to port into GV unless you want to give out a whole new #. Your issue would be call forwarding to GV on the office phone. Home you should be able to CCF as long as it's a local #.
