I've had this happen before, the app drawer doesn't show everything in it. Usually only happens to me after the first boot of making a data.img, next boots are usually fine...
You can always create a shortcut for the icon, I've only see the app disappear from the app drawer... it's never denied you from creating a shortcut to it on one of your homescreens.

I'm not sure what the problem is... what else is different other than the system.ext2 file in that cab you installed? Maybe a newer kernel broke it? I seriously doubt it, but I guess it's possible.
