I did a hard reset, and no dice. I don't think it is a hardware issue because when I restore to stock it works fine.

it sounds like its an issue with the touch screen.
have you tried installing a completely stock rom ?
you could also try flashing an ruu (you will loose root)
but if you have s-off that should not be a problem.
that way you can start from scratch and then you will know
if its the touch screen or not.
