Well i guess the start to trying to fix this issue would be asking what ROM are you using? Then if you are running a stock ROM, you may be okay by simply doing a hard reset. If that doesn't help then I'd suggest you have a bad device or it's the ROM you're running. Of course there are probably several other options to look at but this is where I would start.
