One thing I noticed (which might be the problem) is that in the ESPT settings (under A-Key), there is no authentication key there. I wonder if there is supposed to be something that makes my phone authenticate over the network. ((shrug)).

since your getting a new phone it's null, but for the others out there, the number you replaced with your phone # may have needed a different number in some of those spots. if you reset back to default (above ## codes) then program the phone following the instructions from your carrier (usually ##yourMSL# )
after that your data should be able to provision succesfully.
