The online form states that it is for Sprint employees that have SERO accounts and are leaving the company, but if you choose option one it will work for anyone that has SERO already. I already received an email from Sprint saying that the change has been made and it will take affect beginning my next billing cycle which is about one week from now.

Yes, I would like the link. I emailed ecare last week asking to do this and they said no, so if there's an online form I can try I would certainly appreciate it.
