I'm just going to throw this out there, but you are provisioned right? hopefully someone else with T-Mobile will chime in and say something, maybe it's something that's wrong with the network in your area? maybe you are installing something that's breaking your T-Mobile settings? even if you are provisioned i would run the provision again just to make sure.
