Are you overclocking? If so back her down a notch (to 998 Max) and see if it continues.
If it continues or if you arent overclocking then flash the stock kernel (wipe cache and dalvik first) and see if it happens. My guess would be the kernel.

EDIT: Actually if you arent overclocking then just try wiping cache and dalvik and see if it continues on #8, THEN try the stock kernel. |
Kyler, keep us posted on your findings with this. I have not yet tried my HDMI cable on this ROM/Kernal yet, but I do use HDMI and would like to know what is causing yours not to work. Actually, I will test this tonight myself and let you know my findings as well.
