I've always had weird results with ICS over USB on every phone I've used (Apache, Titan, Touch). As weird as it seems, I get the best results when I follow a strict startup sequence:

-Initiate ICS
-Plug USB cable into phone
-Plug USB cable into computer

If I plug the cable into the computer first, I never get a connection. If I plug the cable into the phone before ICS is running, same thing. Could just be a series of weird coincidences, I can't be sure. But that sequence seems to work for me virtually every time. Not that I use USB that much. WMWiFiRouter all the way...
