I also was using a phone canvas 3.5+ before and only switched to this one to see if the dialer was the issue. Since the issue remains I've left this dialer in for the time being as I've grown to like it.
I'm not finding and duplicate files... Have to go to work, but I'll plug away at it some more when I get home tonight. Thanks for you help guys! I really appreciate it!

oh okay (about the phonecanvas).
Well, I have the time to reply now, so good luck with this later on tonight. And, btw, these are just some observations because I can't pinpoint whats going on either.
-Note: Module duplicate Warning comes before the package and the parsing errors and overwrites come after the package is listed

-OEMDrivers has an sms_providers.dll that always overwrites the one in the SYS (I delete the one in the SYS)

- I turned HomeService.dll into a module cause that's how it is originally in Sense anyway.
After writing all this... I noticed that you may not be using a few of the newer HTC OEMs. I've found them more solid for 2016.
