I know this is old, but I found it by searching, and so, if someone else does the same, they can benefit from my effort.

The SIM Card holder does not contain any pin information. However, it does contain the number for Sprint Worldwide Seervice, 1-817-698-4199, option 2.

They informed me that the PIN would be either (A) 0000, (B) last 4 of your phone number, or (C) somewhere on the documents included with the phone. It was immediately clear to me that it wouldn't be B or C (no documents with my phone; phone wasn't shipped activated, so it didn't know my phone number), but A worked (for PIN1, at least) -- it was 0000.

Unfortunately, SIM lock doesn't seem to do a thing in the US since the connection is to Sprint over CDMA (not too surprising). Oh well. Curiosity sated.
