does the phone portion work? are you able to make and receive calls?

for the apk, if it's not set correctly, the apn will disappear. make sure all the settings in the apn are correct per:

You should see an entry with Name: Android (or CDMA Generic), and APN: internet. If this entry is not active, activate it.
