If you have nothing in the upper left corner, and nothing in the registry, then it's a problem with your phone, X found a fix for it, but I'll have to ask him again what it was.
EDIT: According to X, it takes manually updating the PRI and PRL on the phone.

mine says Sprint, doesnt look like spacing is at play, tried it with full signal from my Airave, as well as a couple of blocks from my house with full Sprint signal (even tried it when Roaming), still wont AutoProv for me
I had run the PRI/PRL/Profile update about a month+, will try again, then hard reset, then let you know
