When your phone first comes up to the home tab of Manila, what is displayed in the upper left hand corner?? Also check HKLM\System\State\Phone\Current Operator Name, that's what the radio and NV Ram report back to WinMo, it's also how I determine what to auto-prov your device to. If you have something there other than (without the quotes):
"Verizon Wireless"
"Sprint"
"TELUS "
" TELUS"
"U.S. Cellular"

Let me know, and I'll add it to the program, and yes Telus is really that gay and puts 7 spaces either after or before their name.

If you have nothing in the upper left corner, and nothing in the registry, then it's a problem with your phone, X found a fix for it, but I'll have to ask him again what it was.

EDIT: According to X, it takes manually updating the PRI and PRL on the phone.
