The security policy is pushed after I setup my exchange account. This requires an alphanumeric or simple numeric password.

I think I was able to reproduce the issue again after flashing.
Flashed rom, configured outlook to connect to Exchange server which required a numlock (used 6 digit code and time set to 30 min).
Went to home page, chose edit lock settings layout..changed layout to my liking and then chose cookie hometab editor, clicked misc settings (no changes made), clicked appts & tasks (turned off tasks), clicked lock settings (no changes made). Exit out of CHT to save settings and reset the device. On a reboot, you will be presented with a lock screen (blue buttons on white background) before booting into home screen and the codes will not work. Reflash is the only way to recover.
