You need a graphics program that will write .png files and some talent. When you get a program that can create .png files open an existing clock font and look at it to see correct pixel size etc., and create away.
You also have to know how to make the background correctly. I have edited existing clock fonts using PhotoShop Elements and pasted them over originals to keep the same background (i.e. transparent etc.).
I don't know that much about how to create them from scratch but I do know this is something that can be taught in a few forum posts.
