I did a little research on the forums here, and learned about the VZW Provisioning Cab (even though it was my understanding that the ROM auto-provisions). Before doing anything, I tested voice and text and data...all 3 worked, even though I couldn't *228. I finally found the Provisioning section of the ROM, and saw that it said Telus. I changed it to Verizon and hit "Commit" and the phone rebooted. After reboot, I tried *228 again, and again it failed.
I called it a night since it was getting late. I went in to work this morning, and did a bit more research on it, hoping to fix it. I discovered somebody saying to try ##RTN#. Tried that, and it failed. I then tried *228, 1 again, and this time it successfully programmed! I have no idea why or how though, as my signal strength at home is 4-5 bars, and I am lucky to get 1 bar at work.
Any clues as to why this happened?

1st...I would flash the updated VZW 6.5 rom to get the new radio HERE
Typically I flash the new rom, once loaded I *228 and hit 1 to program. After that I clear storage. Once rebooted, it usually auto provisions.
Can't say why it's not working for you, just wanted to post what has worked for me.
