The only thing not clear about that screenshot, under Current Customers, is the "designed for the HTC EVO 4G". Other than that, it sounds just like an Everything Data Plan (voice/messaging/data). Like I've said in earlier threads, I would pay up to $15/mo for 4G, as an add-on to my current Everything Data plan.
Also, if the Evo will be available in non-4G markets, you can't force the customers into a 4G plan, when 4G data isn't available. If you have an Everything Data Plan, you should be able to get the phone. My gut feeling tells me you will be charged for the hotspot app, if you use it, and that's it.
