The State and Notification Broker sets HKLM\System\State\Phone\Current Operator Name, from the values in the NVM and radio. This is the same key that defines what displays as the Operator Name in the upper left hand corner of the screen. Unfortunately, there are some curcumstances where this key is screwed up, and this is why auto-prov doesn't work for everyone. If this is the case, you'll have to try ##RTN#, if that doesn't work, like it didn't for me, you can try *228 option 1 on VZW.
