Did you uninstall the custom slider, then soft reset the device? That should do it. If not, which version of WM are you running 6.5 or 6.1, and which version of manila 2.5 or 2.1? I'm sure there is a stock slider out there for your set up.

Edit: And which ROM, stock or custom (and which one)?
