Deductible will be the same regardless of reason for claim. I wouldn't worry about the flashed ROM in this case. If you went in for warranty repair/replacement you might have trouble but I seriously doubt it for this.
My best advice is just to be honest when making an insurance claim. If you have it, send it back. Any insurance claim through Asurion does have you attest that you are not committing insurance fraud. I can't imagine them ever following up, but still.
If you ever wanted to see if the phone would be repairable, it would be $120 at a Sprint store for screen replacement if they have the parts.
