Lemme guess, the top left clip broke off so the facing doest hold tight to the phone anymore? It's an educated guess cause rthis is exactly what just happened to me. The little clip that hold the top piece flush to the phone broke off, near the power button, so I am in the process of doing an RMA replacement since I am within the 180 days.

For the part number, I used the part number of the case only not the combo pack that I bought, since I am not sending the belt clip back(and they came as 2 separate items in the original bshipment).
