Actually, I was looking to how the end key is defined differently with VZW and Sprint. In the verizon.cab from your v3.2 the red phone (end) key is "properly" programmed as the VZW shut off key. You can tell it's right because shutxp will come up when you do a long press. On your v4.0, the top key starts shutxp on a long press. This is reflective of Sprint's mapping. When I tried linear's gearhead rom (also based on Calk's kitchen and using the same verizon.cab as your v4.0), I figured out that the red phone key changed the sound mode to vibrate with a long press. Another long press put it back in speaker mode. The length of the vibration on your v4.0 is similar but it doesn't always change the sound mode(???).
To make a long story not as long, I was looking at provisioning files this AM. I found one on the MightyROM thread and tried it out. When I didn't load the tjELITE file MMS didn't work (no surprise). When I ran it after your standard provisioning, everything works (so far). I have phone, mms, ssm, correct keyboard, and the favorites key (and other things I may never use). I may try to combine the two files and offer it up for you to include but I just don't understand why someone hasn't come up with a file that does everything yet. BTW, the MightROM thread recommends IMNUTS mms file fix.
For now, I'm going to maintain this setup and see if I have any issues.

Edit: I checked out the location you gave, it has "Always display the Quick List" but I think it gets overwritten somewhere (registry?) by shutxp.

Is this what you are trying to do with with the provisioning cab.
