All I've ever done is to place the theme's TSK file into My Documents (or a folder in My Documents), though I think it can go just about anywhere. Then go to:

Start > Settings > Personal (default tab) > Today

If you don't see your new theme listed as a choice in that screen's menu, then try soft reset. You may also wish to select a certain image file for use as background. Well, at least, that's how I've gotten what I've wanted. Maybe the one you're trying to run is more complex and requires a CAB install?
