OK, so I got a shortcut to show up in the Start Menu by using Total Commander to make a shortcut directly from the EXE and not just copying the .lnk file to the Start Menu folder. Also, you need to do this for FEXPLORE.EXE (File Explorer) if you want a shortcut for the default File Explorer.

So in short, use Total Command to create shortcuts for both IEXPLORE.EXE and FEXPLORE.EXE in the Start Menu folder. The Start Menu folder and exe's are under the Windows directory.

Hope this helps some people out.
