I recently had to have my phone replaced and it would not activate so I had to have Verizon do a manual activation.

Here's what they did:

On the mail phone screen dial ##000000 and talk
This gets you to the Activation Wizard (MSL) where you can see and edit:
MDM
MSID
Home SID/NID #1

I want to switch my phone over to the Boost CDMA network so I think I would have to have access to this screen.
