I had this occur and reloaded back to the original VZW (by choice to be safe) then called #8899, then ##778, Edit, then enter 0 (zero) 6 times for pwd, View Info, A-KEY, Authentication Key, then enter the provided 20 something character code given by the VZW Agent.

Just let them know you did a factory reset and it seemed to have problems connecting to voicecalls. Don't mention anything about other codes/roms. Once the code is entered, you are good to go.
