OK guys need some advice here, I stopped coming here as much so i'm kinda out of the loop.

Today my screen went blank for about 8 hours. I could still get calls but my screen was unresponsive and blank. About a hour ago, someone called and it started to work again. It goes in and out randomly. I am also missing a big chip of chrome above the volume button. Are these a good enough reason for Asurion to replace my phone? I have had coverage from day one.

Also, Do I have to rom my phone back to factory before sending it back in?
