Do you have a SD card inserted? Try removing it and soft resetting a few times to see if it resets reliably each time. If so you may have a corrupt or damaged card. Failing that, are you doing a hard reset / clear storage after you flash? Some say this is a crock...others swear by it....personally I can vouch for the procedure from experience.
