I also had this problem with the Fresh rom and the wifi tether app. You get that screen because USB debugging is not turned on. If it was you would get a menu asking you to allow the app super user access. So, instead you get the black screen with "SU Request"
On my hero I didn't have to reinstall the app. Just go to:
Menu > Settings > Applications > Development > switch on USB debugging
