I've really only noticed similar issues with the 2.5 manila builds since those came off of devices with larger screens. You listed two 2.1 manila builds with the only difference being the WM versions. I havent seen any other posts of this nature. IMO alignment issues with manila 2.1 sounds odd, it's possible a flash to stock and back could be in order. DO you have any manila mods in place that could be causing an issue?
One thing I can suggest is to use the Dpad for the one or two items that are off screen. Not great I know, but if you know there is another button/icon/choice/etc just off screen, the Dpad should take you to it and then press the center button.
I use Total Commander frequently and dont notice any issues, maybe some more details and/or a screenshot would get some more input?
