If hard resets and reflashes haven't fixed the problem, you either have:
1. Conflicting program/hack
2. NAND corruption.

I recommend:

1. Use the Clear Storage option in Start>Settings>System (note: this is BETTER than hard-resetting with the buttons)
2. Reflash the stock ROM
3. Clear Storage
4. Flash custom ROM
5. Clear Storage
6. See if it happens again.

I cannot stress it enough - hard resets by using the hardware buttons are NOT as effective as doing it through Start>Settings>System>Clear Storage
